Are you saying you KNOW you heard it directly, but aprs.fi doesn't show
it as heard direct?

If you receive a packet directly originally sent with say WIDE2-1 and
gate it, aprs.fi (incorrectly) assumes it was originated with WIDE2-2
and therefore you did not hear it direct. Perhaps the cases you mention
are in this category.

The whole WIDE / TRACE / RELAY thing is not uniformly implemented, and
there isn't a guaranteed reliable method of determining number of hops a
packet has had in general. The AX.25 protocol should have included a
mandatory hop count in the header, but it didn't and probably never will
because it is mostly carried by outdated infrastructure that cannot
/will not be upgraded.

I do not use WIDE2-1 during trials where I want to use the aprs.fi reports.
